PORTABLE UNIT AGREEMENT FORM

 

The portable unit agreement form is intended for students who wish to check out WOWL’s portable unit and accessories for FAU events or other events there approved by management.  The equipment is available to all FAU students registered in an organization recognized by the university.  To preserve and regulate the portable equipment, students who seek to check it out must comply to following regulations and forms:

 

These forms are legally binding and have been approved by Student Government and the FAU administration.

FORM A:  You must prove that you belong to an FAU organization. You must also have verification that your event has been approved by campus activities. Please bring the attached form fully completed and signed by an authority who can vouch for you. 

 

FORM B:  The student taking responsibility for the portable unit and equipment must furnish her or his full address, contact information, and social security number.

q  You must schedule the check out date two weeks prior to the event. If the event is in less than two weeks, it is up to the discretion of WOWL managers to allow for use of equipment.

 

There will be a $25/hr fee charged for a dj/attendent plus gas if applicable.  Be aware the fee is not for the equipment but for a WOWL member to dj, advise or watch over the equipment.  You must furnish the dj/attendent fee one week before the event in the form of check, cash, or money order.

q   It is the responsibility of the designated user to arrange for equipment transportation unless the responsibility is taken up by the station.  For example: Request for large vehicle, dolly cart, or golf cart.

 

In the case that damage has occurred to the portable equipment, the contact person or organization will be held liable.  Penalty fees will be determined by the amount of damage and cost of equipment.
